The information that I have provided on this information form is accurate and true to the best of my knowledge. Cooperative Baptist Fellowship representatives are hereby authorized to gather information as it relates to the process for appointment. I release from liability any individuals named on this application that provide information required for an appointment decision to be made. Appointment may be refused or terminated in the event that misrepresentations or deliberate omissions are made on this or future forms and information I may submit. Information forms will be kept on file for six (6) months. Any documents completed or provided during the information and application process become the property of the Cooperative Baptist Fellowship and will not be returned.
